About the Ending the HIV Epidemic program: Provides short-term HIV primary medical care and support services for newly diagnosed individuals and those who have fallen out of care. Once enrolled in care, individuals are transferred to long-term care services and providers.

About the program: Provides long-term HIV primary medical care and support services for those living with HIV who are low-income, uninsured or underinsured.

Client eligibility: Proof of HIV Status, identification, and income.

Residency requirements: Must reside in Harris, Fort Bend, Chambers, Liberty, Montgomery, or Waller Counties.
